Our forms page contains both our Adventure Information Packet and Trip Evaluation Forms.  The documents are writeable PDF’s that can be saved and emailed back to Alaska Alpine Adventures as an attachment.  Please do not hesitate to call us with questions concerning these documents.

Adventure Information Packet - If you have signed up for an Alaska Alpine Adventure, you will need to fill out this form and return it to us no later than 60-days prior to your trip's start date.  Please read everything thoroughly. The information in this packet should answer most of the questions you may have about your trip. All clients of Alaska Alpine Adventures, LLC, are required to complete and sign a liability release, client profile form, and acknowledgment of risk prior to trip departure.
